Birth, Death and Marriage Certificates

To Obtain a Certified Copy of Birth, Death, or Marriage Certificates:

You may send a letter to the Town Clerk’s office, 488 Chapin Street, Ludlow, MA 01056, or use one of the following forms:

• Birth Certificate Request Form
• Death Certificate Request Form
• Marriage Certificate Request Form

The cost is $10.00 per certified copy.

Make check payable to the Town of Ludlow.

Please include a self-addressed stamped envelope.

Note: Before mailing in your request, please contact the Town Clerk’s office to be certain we have this record. Some records are restricted or impounded and access may be denied. To prevent any delays, please enclose a copy of a government-issued ID with your request.

Requirements for Marriage Intention:

• The ceremony must be performed in a city or town within Massachusetts.
• Both parties will need to come into the Town Clerk’s office, 488 Chapin Street, to complete the “Notice of Intention of Marriage.”
• Please have a government-issued ID available.
• The cost is $15.00 (cash or check).
• Individual must be 18 years of age or older to file your marriage intentions; After filing your paperwork, there is a three (3) day waiting period before your license can be issued; Marriage licenses are valid for 60 days front the date of filing your intention paperwork.
